Patient Education Referrals

The Shands Diabetes Self-Management Education Program for Adults

The Shands at UF Diabetes Self-Management Education (DSME) Program accepts referrals from care providers and is covered by most insurance plans. The DSME is an American Diabetes Association recognized education program.

After an individual assessment with a Certified Diabetes Educator (CDE) patients attend a serise of classroom sessions taught by CDEs, who are nurses, dietitians and other health professionals. Most participants improve blood sugar control and on average realize a reduction in BMI as a result of attending this program.

Patients can be referred to the entire program, or specific portions related to their educational needs such as carbohydrate counting and new insulin medication or meter start.

We also recommend referring your patients for an annual follow up session for ongoing support and exposure to best practices in diabetes management.
