For Professionals
The UF Diabetes Center of Excellence provides a series of professional education opportunities for health care professionals who treat patients with diabetes. We start of the year in January with Diabetes Care, a day-long educational program that covers all aspects of diabetes managment. We offer a series of diabetes-related seminars taught by certified diabetes educators where nurses can learn about diabetes managment and new technologies while earning continuing education credits.
If you are a primary care provider who needs to refer a patient to our Diabetes Self-Management Education Program, please fill out the Shands Diabetes Self-Management Education Program Referral Form and fax it to (352) 265-8425.
