What to bring with you

Whether you are a new or returning clinical patient, we suggest that you think about and bring the following to your next clinical visit:

  • Write down all of your questions and bring them with you.
  • Write down or bring all medications and supplements so our clinicians will know what you are taking. (Including vitamins and over-the-counter medications.)
  • Bring your meter(s) and your logbook. You may already have a logbook available to you. If you don’t, here are some easy to use logs available to you:

    This log will help you keep track of multiple daily injections, blood sugars, AND food intake.

  • It is important if you are also aware of your carbohydrate ratios, your “correction factor” and your basal rates, even if you are on the pump.
  • Please confirm the date of your last eye exam prior to your next visit to our clinic.
